Please Complete SLU鈥檚 COVID-19 Vaccine Survey

Dear SLU faculty, staff and students,

Thank you for the outpouring of support yesterday after we announced what we hope will be the start of more opportunities to vaccinate members of our community.

Future COVID-19 vaccine clinics hinge on the success of our first, and we owe a tremendous debt to the clinical and staff volunteers who are making it possible.

In preparation for future clinics, we ask thatall full- and part-time faculty, staff and students.

This survey will allow our team to start building lists of eligible community members for future vaccine clinics.

We are relying on our entire community to accurately identify the phase/tier into which they fall. This helps ensure that the most vulnerable members of our community are prioritized with the limited number of vaccines we currently have available. Please fill it out by the end of the week.

This survey is only for full- and part-time SLU faculty, staff and students . We hope in the future to expand to loved ones, and will keep you updated when that changes.
