COVID-19 Vaccines for Faculty and Staff Household Members

Dear SLU staff and faculty,

Thanks to the generosity of the Missouri National Guard, we are excited to announce that we will soon be able to invitehousehold members (over age 18) of our full- and part-time faculty and staff to get vaccinated, too.

We will be offering the Moderna vaccine.

In order to identify eligible household members, we ask that you. The survey helps us narrow down who fits into which prioritization phase/tier according to.

What you need to know:

We have several clinics scheduled for this week, so please fill out the form, or have your household members fill out the form,as soon as possible.

Reminder: We willnottell you which phase/tier you or your loved ones fit into. Please review the guidance prior to making that discernment. Some of the details from the state are open-ended and we rely on you to determine where you fit based on your personal circumstances.
